Starting the Conversation

When discussing senior care, it’s important to recognize the invaluable support that family members and friends can offer to aging parents or loved ones. Making decisions requires careful planning, and involving family or friends can be critical for a successful transition.

For many seniors, moving into a senior living community signifies not only a decline in their physical and mental well-being but also a permanent loss of independence. This transition to a new environment impacts not only your loved ones but oftentimes friends and family members also. It can be a difficult and emotionally challenging time. When discussing long-term care needs, it’s important to consider factors like finances, health, and interpersonal relationships. We strongly recommend starting these conversations early on.

Initial Consultation

Typically we conduct a thorough assessment over the phone and when necessary, will perform the assessment at the home, rehabilitation center, or hospital. Our thorough intake consultation is designed to fully understand you or your loved one’s physical, psychological, and geographical needs, as well as specifics such as care needs, budget, lifestyle, and family considerations.

Identifying Care Needs

After the initial consultation and assessment, we will determine the most appropriate senior living options using our proven process. These may include assisted living, independent living or memory care communities, residential board & care homes, or in some cases, in-home care. Once identified, we will discuss these options with you or your loved one in detail and provide information on current pricing, availability, and location.

Tour & Selection Process

Next, we will determine appropriate options based on the information obtained during the assessment and make recommendations based on our in-depth knowledge of those communities or homes, taking into consideration the overall needs of the individual and the feedback regarding those that we have previously placed there.

We will then arrange for a private tour of each option. After touring and once a choice has been made, we will offer our assistance in obtaining the necessary medical forms and documentation needed for the transition. We will be with you every step of the way. Ultimately, our goal is to have the individual thrive because they are well cared for and happy.

Physician’s Report for Residential Care Facilities for the Elderly (RCFE)

Admission to any assisted living community requires that a potential resident submit an RCFE (also know as a Physician’s Report or 602) prior to moving in. This form is completed and signed by the potential resident’s primary doctor to assess current functional capacity, mental condition and any other pertinent health-related issues or considerations.
